CHC: Creating Healthier Communities
We build partnerships to address health inequities by fostering trusted community engagement, fundraising and offering programs and services to help support communities where everyone can thrive. Together with partners, we’ve been creating healthier communities for more than 70 years. We exist so that every person in every community has an opportunity to live their healthiest life. In Minnesota, CHC represents 50+ high-impact nonprofits’ work to address the barriers to good health and equity while powering transformative change in the communities in which we live and work. Health equity is going to take all of us—organizations, businesses, and individuals—working together in communities across the country. CHC’s role is to enhance the capacity of nonprofit organizations, connect and convene diverse stakeholders, and harness collective resources and programs to activate collaborations that address root causes and improve the health of our communities.
A gift of $1.00 a week supplies one family with a “First 100 Day Kit” that includes information about autism and resources to help deal with the news of a diagnosis. $2.00 a week will help cover almost 2/3 of the cost of one-hour physical therapy at St. Jude Children’s research hospital. Every dollar raised goes to trusted nonprofits to meet unmet needs in our communities.

Open Your Heart to the Hungry and Homeless
Since 1986, Open Your Heart to the Hungry and Homeless has focused solely on ensuring that Minnesotans have their most basic needs of food, shelter, and safety met. Our guiding principle has always been to give where the need is greatest. We do that by supporting nonprofit organizations assisting Minnesotans experiencing food and housing insecurity, ensuring they have the resources, equipment, and infrastructure necessary to best serve those in need.
For nearly four decades, Open Your Heart to the Hungry and Homeless has had a significant impact in the lives of Minnesotans and in the health of their communities. Each year we support over 200 different crisis and basic needs service agencies, collectively impacting millions of Minnesotans experiencing hardships.
Open Your Heart is unique in that our support extends to programs in communities all over Minnesota, including the Twin Cities metro, and the cities, small towns and rural areas of greater Minnesota. Recognizing that our neighbors have needs that can’t wait, we are nimble and responsive providing quarterly grant rounds and emergency support within days during crisis.
UNCF
Since 1944, UNCF—United Negro College Fund—has helped minority students live the American Dream by helping more than half a million young men and women graduate from colleges across the United States.
Today, UNCF supports more than 50,000 students attending over 1,100 colleges and universities nationwide, including 37 UNCF-member HBCUS (Historically Black Colleges and Universities), thereby providing the opportunity for a college education at an HBCU or any other college or university.
